The New Patient Visit

My last article in the Full Mouth Reconstruction Series<\/a> <\/span><\/span><\/strong>was about the Phone call\/First encounter.<\/strong> Please go back to that article<\/a> for more details on the new patient phone call. During the new patient phone call, the new patient shared with you what they are looking for and you have shared with them a little about your practice philosophy.<\/span><\/span> You have also given them hope and possibility. <\/span><\/span><\/strong><\/p>\n

The best scenario is that you schedule them within a week or two of the phone call for their new patient visit. <\/strong>We email or mail them new patient paperwork. We used to send a welcome packet via regular mail… There are so many ways to do new patient visits and most of them work. Pick the way that makes the most sense to you and try it for a while. The patients will accept whatever you do and you can always change it if you need to. <\/span><\/span><\/p>\n

One of my favorite trainings I’ve had on the new patient visit is Bob Frazer’s New Patient workshop. I also like the way Frank Spear does his new patients and Pankey teaches yet another way.<\/strong><\/em> I’ve done most of them just like they’re taught for a while and have ended up adapting them\/making them into my own to fit my practice and schedule. Most patients come in through a comprehensive exam as I describe below. Very few come in as an emergency exam and very few come in through hygiene. We have also had a few come in for a 30 minute consultation and meet the doctor. <\/span><\/span><\/p>\n

Most of our patients come in through a comprehensive exam.<\/strong> <\/span><\/span>Before the patient comes in, my assistant reviews the notes from the phone call.<\/strong> <\/span><\/span><\/p>\n

Their first appointment begins with a greeting from the person who spoke with them on the phone.<\/em> Then they are brought back to the consultation room for a 15-20 minute interview to find out more about them and what they are looking for. <\/span><\/span><\/p>\n

I come into the consult room when my assistant notifies me that she’s ready.<\/em> She gets me caught up on what she and the patient have been talking about, I ask more clarifying questions, and I review the medical and dental history. Once the patient has had the opportunity to tell us and ask us everything they can think of, I recommend the records I think they’ll need based on the conversation we’ve just had (typically at this point, it will be an FMX and a set of photographs). <\/span><\/span><\/p>\n

My assistant and the patient will go to the operatory for records and I will join them<\/em> when the records are finished for a dental health exam, which is a tooth and periodontal exam. <\/span><\/span><\/p>\n

I will ask preliminary TMJ questions and sometimes go ahead and finish the TMJ\/occlusal exam and esthetic evaluation that day, depending on time and the patient’s desires and needs (as well as the patients awareness of their needs). <\/span><\/span><\/em><\/p>\n<\/blockquote>\n

If there is low awareness and\/or a lot of needs, I spend more time with them helping them become aware of their problems.<\/strong> <\/span><\/span><\/p>\n

The best way to do this is to bring them back for another appointment to finish the exam and review their photographs.<\/em> <\/span><\/span><\/p>\n

This second appointment can be done as the next step or you may want to have the patient come in for hygiene, go for some specialist consults, or do phase one treatment first. I generally like to do the second visit as soon as possible. Sometimes we will recommend mounted models at this second visit. Depending on the size of the treatment plan and the patient’s readiness and awareness, the next step will be to have the patient come back for a consultation to talk about a recommended treatment plan. <\/span><\/span><\/p>\n<\/blockquote>\n

As an aside, I have learned that although data collection is important, we can get all the data we need over a few visits—we don’t have to get it all in the first visit<\/span>. <\/span><\/span><\/strong><\/p>\n

Finding out the patient’s understanding of where they are, how they got there and where they want to go are the most important things.<\/em> Much of this learning comes through listening skills and emotional intelligence.<\/span><\/span> <\/span><\/span><\/strong><\/p>\n

Collaboration with the patient is the best part of the whole thing and can make it feel like they are literally asking for treatment rather than you telling them what they need.<\/span> THIS IS THE KEY TO BUILDING RAPPORT AND RELATIONSHIP WITH YOUR PATIENTS. THEY ARE INVOLVED IN THE PROCESS. <\/span><\/span><\/strong><\/p>\n

Here is the data we need, depending on the case:<\/strong> FMX, pano, photos, and mounted models as well as documentation of periodontal charting and tooth, soft tissue exam, TMJ, muscle and occlusion exam. <\/span><\/span><\/p>\n
